Terms and Conditions of Service

These Terms and Conditions apply to all services provided by Peak Occupational Therapy ("we", "us", "our"). By booking and attending an appointment, you ("the Client", "you") agree to these Terms and Conditions.

1. About Peak Occupational Therapy

Peak Occupational Therapy is an independent hand therapy service providing specialist assessment, treatment, advice, rehabilitation, and education for hand and upper limb conditions. Services are delivered by a HCPC-registered Occupational Therapist in line with professional standards and scope of practice. While all care is provided with reasonable skill and care, no specific outcome can be guaranteed, as individual response to therapy varies.

2. Appointments and Booking

Appointments may be booked by email, telephone, or online where available. Appointment length, format, and location will be agreed at the time of booking. It is your responsibility to attend at the agreed time and provide accurate contact and medical information.

3. Fees and Payment

Fees are confirmed at the time of booking and may be reviewed periodically. Payment is required as agreed at booking. If treatment is funded by private medical insurance, you are responsible for confirming authorisation and paying any shortfall. You remain personally liable for all fees.

4. Cancellations, Missed Appointments, and Lateness

Appointments cancelled with less than 24 hours’ notice may be charged in full. Missed appointments without notice will be charged at 100% of the appointment fee. Late arrival may result in a shortened session with the full fee still applying.

5. Home Visit Terms

For home visits, you are responsible for ensuring the environment is safe, clean, and suitable for therapy. Any known risks must be disclosed in advance. Peak Occupational Therapy reserves the right to decline or terminate a visit if conditions are unsafe. Home visit fees include travel unless otherwise stated.

6. Consent and Clinical Information

By attending treatment, you confirm that all information provided is accurate and that you consent to assessment and treatment. Consent may be withdrawn at any time.

7. Splints, Equipment, and Materials

Where splints or equipment are provided, additional charges may apply. You are responsible for using equipment as instructed. We are not responsible for damage caused by misuse or failure to follow advice.

8. Remote Appointments

For remote consultations, you must ensure a private and suitable environment. Advice is based on information provided and what can reasonably be assessed remotely. Remote sessions may not be appropriate for all conditions.

9. Children and Young People

For clients under 18, consent must be provided by a parent or legal guardian. A responsible adult must usually be present during sessions. Where appropriate, the views of Gillick-competent young people will be respected.

10. Safeguarding

We have a professional duty to safeguard children and vulnerable individuals. Information may be shared without consent where there is a safeguarding concern or legal obligation.

11. Confidentiality and Data Protection

All personal data is handled in accordance with UK data protection legislation and professional standards. Please refer to our Privacy Notice for full details.

12. Complaints

If you are dissatisfied with any aspect of the service, please raise this directly with Peak Occupational Therapy in the first instance. You may also raise concerns with the Health and Care Professions Council if appropriate.

13. Limitation of Liability

Nothing in these Terms limits liability for death or personal injury caused by negligence. Liability is otherwise limited to the extent permitted by UK law and covered by professional indemnity insurance.

14. Governing Law

These Terms and Conditions are governed by the laws of England and Wales.

15. Changes to These Terms

We reserve the right to update these Terms and Conditions. The version in force at the time of booking will apply.
